This form is used by UNH departments for support/troubleshooting, training, feature requests, and creating event registration for public and paid events, including professional development, corporate training, youth programs and summer camps.
Important: When requesting the creation of a course, section, or conference please ensure that your ticket contains the full FOAPAL (Fund, Org, Account and optional-Activity Code) as well as the Program Office and Costing Unit (department category).
