PSU & UNH: Student Experiences of Learning (SEL) - How to Import Student Evaluation of Teaching Template Qualtrics

Task: This article describes how to create a Student Survey of Teaching survey from a template within Qualtrics.

Instructions:

Creating the Survey Form

  1. Navigate to Qualtrics - https://survey.unh.edu - and log in with your username/password.

NOTE FOR FIRST TIME USERS:  You will be asked if you have an existing Qualtrics account (perhaps from another institution). If you do, and ONLY if you would like to merge it with your UNH Qualtrics account, please select Yes. The majority of users will select No to this question.

Here are the steps you will follow for a new account (answering No).

Click Create New User - a window will display with the following fields:

  1. Username
  2. Email - fill in the field with your email address using your alias (ie. jjsmith@unh.edu)
  3. Fill in first and last name
  4. Terms and Conditions will display - click Agree
  5. You should be brought to the Projects page so you can begin creating a survey project.
  6. Click the blue Create New project button (located on the lower left side of the page)
  7. Click on Survey under From scratch

  1. Select Get Started in the lower right of the page
  2. Name the Survey
  3. How do you want to start your survey?  select "Use a Survey from your library"
  4. Library, select UNH - All Users
  5. Survey, choose, All Projects and Programs and then "SET Standard Questions Template pre-COVID"

  1. Click "Create Project

Modifying the Survey Form

  1. The survey should already be set to anonymous, but it is important to double-check by clicking Survey Options > Security and selecting Anonymize Response at the bottom of the screen
  2. In the introductory paragraph of the survey, add your course name, section name, and your name in the fields provided.
  3. Click the green Publish button in the upper right to make the survey viewable.
  4. To obtain an anonymous link to distribute the survey click Distributions on the menu and click Anonymous Link (a single reusable link) from the left.
  5. Right-click on the link to copy it.
  6. Share the link by emailing to the class and/or posting in Canvas.
  7. Repeat this process from the beginning if you wish to administer this survey to students in a different course.

 

Using the same survey for multiple sections:

If you would like to use the same survey for multiple sections, here is how you can create a selection box to identify the section

Once you have opened the project

  1. Mouse over the question to where you would like to place your new Section question
  2. You will see a Blue plus sign (+) on the top and bottom left corners.  Top right corner will add the question above the current question, the bottom left will add the new question below the current.  Click the appropriate plus sign. 
  3. Once the new question has been added, modify "Click to write the question text"
  4. On the left, Under Question Type, add in as many choices as needed, system defaults to 3 but you can add more
  5. Under Format Choose "Dropdown"
  6. You now have the option to Edit the choices at the question level by adding in the Course and Sections

Creating the survey

Outcome:

Users should now be able to use Qualtrics to set up a course evaluation and send to students.
